    Exclamation Medical Professionals (PCM's, Dr's, etc) Making False Statements in Medical Records

    Reviewing my entire military medical record, I have noticed some discrepancies....

    Probably the most blatant discrepancy is documentation from multiple PCM's and Specialists at multiple bases stating how much time they spent with me, the patient.

    After the diagnosis of whatever reason you are there, there is a spot for documentation of what was discussed with you and how long they spent face to face with you. Almost every documented spot has 20 or more minutes annotated.

    I have never had a PCM spend 5 minutes with me face to face much less 20. And VERY few specialists spent 10 minutes face to face.

    Another is, the record repeatedly states things are discussed at length with the patient. Again, if I am lucky, I get a full minute with a PCM...nothing discussed "at length."

    Then there are things I have mentioned to the PCM's that are not in the records...specifically about not wanting to be on drug X to begin with much less long term because I am concerned about long term health/side effects...rather, they just noted 'non-compliant' or patient stopped taking meds. There are other things that have been mentioned too that are not in my records.

    The old saying use to always be get a copy of everything...the 21st century saying might need to be, record every visit with a smartphone or other camera.

    After retirement going through records, back when I was on
    Profile for knees, Bursitis, capt stated he suspected malingering,
    Not to me of course. He was mad previous pcm put me on
    Profile with no diagnosis and mad I didn't have a serious structural knee issue,
    Even though I was in constant pain and had previous 100s on test.

    I recently went over my medical record also and noticed the same thing about how much time was spent with me. If it's a 10 minutes appointment they'll actually only spend 10 minutes with you. I also noticed that on something things, especially new medications, it says the doc went over very specific things about the new med. That's never happened to me. It's always been the pharmacy that's gone over things with me. However, the record was actually complete. Nothing was missing from what I could tell which was a huge surprise for me.

    Funny tidbit about how ineffective these doctors can be. I was seen in 2006 for a problem. The doctor at the time diagnosed me and sent me to physical therapy. Fast forward 6 years and I'm still being seen for the same problem. I actually had surgery to correct it after an ortho told me what it was... the same thing the doctor in 2006 said it was. So I spent 6 years in pain and on and off medications and in and out of physical therapy for something that could have been corrected. Guess I got what I paid for.

    I know that all too well myself. Spent 2 years in pain for something that my initial X-rays pointed to as a potential problem but the radiologist said what was noted was likely not the cause of my pain. It took 2 years of endless specialists and other visits and a Patient Advocate complaint to finally get resolution. It required surgery.
